Type 1 diabetes (T1D) is a chronic autoimmune disease in which T-cell-mediated destruction of pancreatic beta cells results in absolute insulin deficiency. Approximately 1.9 million Americans live with T1D, including about 244,000 children and adolescents. Onset typically occurs in childhood or adolescence but can occur at any age, often presenting with polyuria, polydipsia, weight loss, and diabetic ketoacidosis in new-onset disease. Diagnosis is based on hyperglycemia (fasting glucose, HbA1c, or OGTT) with supporting evidence of islet autoimmunity (GAD65, IA-2, ZnT8, insulin autoantibodies) and low/absent C-peptide. Lifelong exogenous insulin is required; modern management combines basal-bolus regimens or insulin pumps with continuous glucose monitoring and, increasingly, automated insulin delivery (closed-loop) systems. Long-term complications include retinopathy, nephropathy, neuropathy, and cardiovascular disease; tight glycemic control and cardiovascular risk-factor management reduce these risks. Acute complications include DKA and severe hypoglycemia.
VX-880 requires immunosuppression; VX-264 uses an encapsulation device to avoid it. Early VX-880 data showed insulin independence in some patients. This is potentially the most transformative T1D approach since insulin itself. Competition: CRISPR Therapeutics, ViaCyte (Vertex acquired). The stem cell-derived islet approach could address the donor shortage for islet transplantation.
