Disease Overview

Rheumatoid arthritis (RA) is a chronic autoimmune inflammatory disease characterized by symmetric polyarthritis, synovial inflammation, and progressive joint destruction driven by autoreactive B cells, T cells, and inflammatory cytokines. In the United States, RA affects approximately 1.3-1.5 million adults, with an age-standardized incidence of approximately 12-13 cases per 100,000 person-years and female-to-male predominance of 2:1 to 3:1. Disease typically peaks around age 60. Current standard of care includes conventional DMARDs (methotrexate), biologics targeting TNF (adalimumab, etanercept, infliximab), IL-6 (tocilizumab), B cells (rituximab), T-cell costimulation (abatacept), and oral JAK inhibitors (tofacitinib, baricitinib, upadacitinib). Despite this armamentarium, approximately 20-40% of patients fail to achieve adequate disease control and cycle through multiple advanced therapies. Refractory RA represents the primary unmet need addressed by deeper B-cell depletion strategies such as BCMA/CD19 T-cell engagers and CAR-T, which target autoantibody-producing plasma cells that escape anti-CD20 therapy. KT501 enters this refractory-RA landscape as a first-in-human tri-specific T-cell engager.

Competitive Landscape

Current RA biologic/targeted-synthetic DMARD landscape is crowded but the refractory segment is underserved.
