Disease Overview

Systemic mastocytosis is a rare clonal mast cell disorder driven in >95% of cases by the somatic KIT D816V gain-of-function mutation, producing unchecked mast cell proliferation, tissue infiltration and episodic mediator release. The World Health Organization classifies SM into indolent SM (ISM, ~85-90% of cases), smoldering SM, and advanced SM (AdvSM) comprising aggressive SM, SM with an associated hematologic neoplasm, and mast cell leukemia. Even in indolent disease, symptoms are often debilitating: flushing, pruritus, gastrointestinal cramping, anaphylaxis, bone pain, fatigue and neurocognitive disturbance. Advanced SM carries a dismal prognosis with median survival historically measured in months to a few years. Diagnosis requires bone marrow biopsy with CD25/CD2/CD30 mast cell immunophenotyping, serum tryptase and KIT D816V testing by droplet digital PCR. Until avapritinib, therapy was limited to supportive antihistamines, cromolyn, omalizumab and cytoreductive agents (midostaurin, cladribine, interferon-alpha) with modest durable responses. The arrival of selective KIT D816V inhibition has transformed the field and expanded pharmacologic optionality from palliation toward disease modification in both indolent and advanced phenotypes.

Competitive Landscape

The systemic mastocytosis treatment landscape has been transformed by selective KIT inhibition. Ayvakit/Ayvakyt (avapritinib, Blueprint/Sanofi) is the first and only therapy approved in both advanced SM (FDA 2021) and indolent SM (FDA May 2023), delivering rapid mast cell burden reduction and symptom control via potent selective KIT D816V inhibition. Competing KIT programs include bezuclastinib (Cogent Biosciences, selective KIT D816V inhibitor, Phase 3 PEAK/APEX/SUMMIT studies with topline data expected 2025-2026) and elenestinib (Blueprint's next-generation KIT inhibitor designed for improved CNS exclusion, Phase 2/3 HARBOR). Earlier-generation multi-kinase inhibitor midostaurin (Rydapt, Novartis) remains approved for advanced SM but has been largely displaced. Supportive/adjunctive agents include omalizumab (Xolair, Novartis/Genentech, anti-IgE), cromolyn sodium, H1/H2 antihistamines and cladribine. BLU-808 (wild-type KIT inhibitor) from the acquired Blueprint pipeline addresses adjacent mast-cell-driven diseases (chronic urticaria, chronic rhinosinusitis with nasal polyps, allergic asthma) where selective wild-type KIT inhibition could rival or complement IgE- and cytokine-directed biologics such as omalizumab, dupilumab and tezepelumab.
