Sensorineural hearing loss affects approximately 37.5 million U.S. adults; congenital sensorineural hearing loss occurs in roughly 1-3 per 1,000 live births in the United States. OTOF gene mutations cause autosomal recessive sensorineural hearing loss type 9 (DFNB9), characterized by profound prelingual deafness due to defective synaptic transmission at the inner hair cell ribbon synapse. OTOF mutations are estimated to account for 1-8% of congenital severe-to-profound sensorineural hearing loss, translating to approximately 200,000 affected patients globally and several thousand new pediatric cases per year. Until 2023 there was no pharmacologic intervention; cochlear implants represent standard of care, with newborn hearing screening enabling early identification. Akouos's AK-OTOF (now LY3884961) is an AAVAnc80 gene therapy delivering full-length human OTOF cDNA via intracochlear administration. The asset competes within an emerging genetic hearing loss class. The first-in-human dose in October 2023 produced restoration of hearing within 30 days in an 11-year-old patient, validating proof of concept.
