Bold immunology pivot on a Fc-silent anti-CD40L; upstream master switch, early stage

Sjogren's disease is a chronic systemic autoimmune disorder characterized by lymphocytic infiltration of exocrine glands causing dry eyes and mouth, with systemic manifestations. No approved disease-modifying systemic therapy addresses the underlying immunology for most patients. Innovent plans to initiate a Phase 2 trial of IBI355 in Sjogren's disease in China by early 2027.
Dazodalibep (Amgen) is the most advanced anti-CD40L in Sjogren's (Phase 3). Other mechanisms in development include nipocalimab (J&J, FcRn) and ianalumab (Novartis, anti-BAFF-R). IBI355 competes on its Fc-silent safety profile within the CD40L class.
IgG4-related disease is a rare, immune-mediated fibro-inflammatory condition that can affect multiple organs (pancreas, salivary/lacrimal glands, kidneys, aorta). There is high unmet need for targeted, steroid-sparing therapies. Spero plans to start a Phase 2 trial of IBI355 in IgG4-RD in Q2 2027.
The anti-CD40L class carries a cautionary history: first-generation antibodies ruplizumab and toralizumab were halted due to Fc-mediated thromboembolic events. Newer entrants include dazodalibep (Amgen/Horizon, anti-CD40L, Phase 3 in Sjogren's) and frexalimab (Sanofi, anti-CD40L, in MS and other autoimmune indications). IBI355's Fc-silent engineering is the key differentiator versus the class's historical safety liability.
