Chronic hepatitis C is a blood-borne RNA-virus infection that drives progressive liver fibrosis, cirrhosis, and hepatocellular carcinoma over decades. In 2008, the standard of care was 48 weeks of pegylated interferon plus ribavirin, achieving sustained virologic response in only ~50-60% of genotype-1 patients with significant tolerability problems. The unmet need was acute: a large prevalent pool, slow-onset disease, and a clearly inadequate backbone — which made direct-acting antivirals targeting NS3/4A protease, NS5A, and NS5B polymerase the most contested R&D battleground in infectious disease.
The 2008 HCV pipeline was already crowded with NS3/4A protease and NS5B polymerase inhibitor programs. Vertex's telaprevir and Schering-Plough/Merck's boceprevir were leading the protease class toward 2011 approvals. On the polymerase side, Pharmasset's nucleotide programs (PSI-6130, PSI-7851 — the precursor to sofosbuvir) and Roche/Pharmasset's R7128 were the highest-profile assets, alongside non-nucleoside inhibitors from Idenix, ViroPharma, Anadys, and Genelabs itself. Genelabs brought a portfolio of HCV NS5B non-nucleoside polymerase inhibitor compounds — including a Novartis-partnered series — and a late-stage hepatitis E candidate already partnered with GSK. For GSK, which had under-invested in HCV relative to peers, the $57M deal was a bolt-on shot on goal: Genelabs shares had collapsed to $0.23 before the bid, and the $1.30/share offer represented a ~430% premium but a tiny check size. Strategic outcome was modest: none of the Genelabs-derived NS5B compounds reached the market, and the eventual HCV cure wave was won by Gilead's Sovaldi/Harvoni (sofosbuvir-based) and AbbVie's Mavyret — making the Genelabs acquisition a low-cost option that ultimately did not pay off.
