Disease Overview

Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized pathologically by loss of dopaminergic neurons in the substantia nigra pars compacta and accumulation of alpha-synuclein in Lewy bodies. It affects approximately 1 million people in the United States, with incidence rising sharply after age 60. The cardinal motor features are bradykinesia with rest tremor, rigidity, and postural instability; onset is typically asymmetric. Non-motor features — hyposmia, REM sleep behavior disorder, constipation, depression and anxiety, cognitive impairment, autonomic dysfunction, and hallucinations — often precede motor symptoms by years and drive much of the disease burden over time. Diagnosis is clinical, supported by response to dopaminergic therapy and, when needed, DaT-SPECT imaging. Management is symptomatic and multimodal: levodopa/carbidopa remains the most effective symptomatic therapy, complemented by dopamine agonists, MAO-B and COMT inhibitors, amantadine for dyskinesia, and advanced options including subcutaneous or intestinal levodopa infusion, apomorphine, and deep brain stimulation for motor fluctuations. Multidisciplinary care including physical, occupational, and speech therapy, and management of cognitive, psychiatric, and autonomic non-motor features, is essential.
