15-prostaglandin dehydrogenase (15-PGDH) is the principal enzyme that degrades prostaglandin E2 (PGE2), a key signaling molecule for stem cell maintenance and tissue repair. Pharmacologic 15-PGDH inhibition raises endogenous PGE2 in damaged tissues, promoting regeneration of intestinal epithelium, lung parenchyma, hematopoietic stem cells and other compartments. The target indications collectively represent substantial unmet need: ulcerative colitis (UC) affects approximately 900,000 US patients with annual incidence of 8-12 per 100,000; Crohn's disease adds another 780,000 US patients; idiopathic pulmonary fibrosis (IPF) affects approximately 100,000 Americans with median survival of 3-5 years post-diagnosis. The global IBD market exceeded $20B in 2021 with TNF inhibitors (Humira, Remicade, Simponi), integrin antagonists (Entyvio), IL-12/23 (Stelara), IL-23 (Skyrizi, Tremfya, Omvoh), JAK inhibitors (Xeljanz, Rinvoq) and S1P modulators (Zeposia, Velsipity).
