Synovial sarcoma is a rare, aggressive soft-tissue sarcoma driven by a pathognomonic SS18-SSX gene fusion, most often arising in the extremities of adolescents and young adults aged 15-40. About 800-1,000 new cases are diagnosed annually in the US; metastatic/unresectable disease has historically had limited durable options after doxorubicin- and ifosfamide-based chemotherapy fails.

Competitive Landscape

Standard care for advanced synovial sarcoma at deal time was anthracycline/ifosfamide chemotherapy, with later-line use of pazopanib (Votrient, Novartis/GSK) for soft-tissue sarcoma and trabectedin (Yondelis, Janssen/PharmaMar) for liposarcoma/leiomyosarcoma — none with durable disease control in synovial sarcoma. NY-ESO-1 is expressed in roughly 70-80% of synovial sarcomas, making it an attractive cell-therapy target, and Adaptimmune's affinity-enhanced TCR (later afami-cel/Tecelra) showed a 50% response rate (6/12) in early synovial-sarcoma cohorts. The expanded February 2016 GSK collaboration accelerated NY-ESO toward registration trials in synovial sarcoma and exploration in MRCLS, with up to eight proof-of-principle combination studies (including checkpoint inhibitors) and added incremental milestones on top of the June 2014 master agreement. The deal validated TCR cell therapy in solid tumors years before the August 2024 Tecelra approval (the first engineered cell therapy in a solid tumor) and shaped the pivotal SPEARHEAD-1 trial that ultimately supported registration after GSK returned the program to Adaptimmune in 2021.

Myxoid/Round Cell Liposarcoma (MRCLS)

Myxoid/round cell liposarcoma (MRCLS) is a rare adipocytic soft-tissue sarcoma defined by FUS-DDIT3 (or rarely EWSR1-DDIT3) translocations, accounting for roughly a third of liposarcomas. Like synovial sarcoma it expresses NY-ESO-1 at high frequency, making it an explore-worthy second indication for SPEAR T-cell therapy.

Competitive Landscape

MRCLS treatment at deal time relied on doxorubicin-based chemotherapy in advanced disease, with trabectedin (Yondelis) holding a particular niche given its activity in translocation-driven liposarcomas, and eribulin (Halaven) approved in 2016 specifically for liposarcoma. The expanded GSK-Adaptimmune agreement specifically called out exploration of NY-ESO SPEAR T cells in MRCLS alongside the synovial-sarcoma registration push, positioning TCR therapy as a potential first-in-class option in a translocation-driven solid tumor with limited durable systemic options.
