Huntington's Disease

Disease Overview

Huntington's disease (HD) is a fatal autosomal-dominant neurodegenerative disorder caused by a CAG repeat expansion in the HTT gene that produces a toxic mutant huntingtin protein, leading to progressive motor, cognitive, and psychiatric decline. Onset is typically in midlife and median survival is 15-20 years from symptom onset. There is no disease-modifying therapy; the symptomatic standard of care addresses chorea but does not slow progression.

Competitive Landscape

At deal signing in February 2018, no disease-modifying therapy existed for HD. The competitive set was dominated by Roche/Ionis's tominersen (IONIS-HTTRx, an allele-non-selective antisense oligonucleotide), which had just completed a Phase 1/2a trial showing dose-dependent mutant huntingtin lowering and was advancing to Phase 3 GENERATION HD1. Approved symptomatic agents included Teva's Austedo (deutetrabenazine) and Lundbeck's Xenazine (tetrabenazine) for chorea, plus off-label antipsychotics and SSRIs. Wave's WVE-120101 and WVE-120102 differentiated as the first stereopure, allele-selective antisense oligonucleotides designed to lower mutant HTT while sparing the wild-type protein, theoretically improving the safety margin versus tominersen. The Takeda partnership validated stereopure ASO chemistry and gave Wave non-dilutive capital plus a global commercialization partner across HD, ALS, FTD, and SCA3. The deal mattered because it positioned a clinical-stage challenger against Roche/Ionis at the moment HD was emerging as a tractable genetic-medicine target. By 2021 both first-generation Wave HD candidates were discontinued for failing to lower mHTT, and tominersen's Phase 3 was halted the same year, resetting the field around next-generation programs including Wave's WVE-003.
