The NLRP3 inflammasome is an intracellular innate-immune sensor whose chronic activation drives IL-1β/IL-18 release and downstream tissue damage across cardiometabolic (gout, atherosclerosis, NASH), gastrointestinal (IBD), and neurodegenerative diseases. Until 2018-19 the axis had been validated clinically only via downstream IL-1β blockade (canakinumab/CANTOS), making selective upstream NLRP3 antagonism a high-value, broadly applicable mechanism.
By April 2019 the NLRP3 antagonist field was nascent but rapidly heating up. Novartis already owned Ilaris (canakinumab, anti-IL-1β) and the CANTOS cardiovascular dataset proving anti-inflammatory therapy could reduce MACE — but CANTOS missed commercially because canakinumab was IV/biologic and expensive.
