Exocrine pancreatic insufficiency (EPI) is a malabsorption syndrome caused by inadequate pancreatic enzyme secretion (lipase, amylase, protease) leading to fat maldigestion, steatorrhea, weight loss, and fat-soluble vitamin deficiencies. Approximately 800,000-1,000,000 U.S. patients have clinically significant EPI driven primarily by chronic pancreatitis (40-50%), pancreatic resection or duct obstruction (15-20%), cystic fibrosis (CF, ~30,000 U.S. patients almost universally affected), pancreatic cancer, and post-Whipple surgical states.
