Cataract — clouding of the eye's natural lens — is the leading cause of reversible blindness globally and is treated almost exclusively via surgical replacement of the lens with an intraocular lens (IOL). Refractive surgery (LASIK and lens-based) corrects myopia, hyperopia and astigmatism. Consumer eye health spans contact-lens care, dry-eye products, and OTC ophthalmics. Combined, the segments serve a large, aging, predominantly elective patient population.
The ophthalmic surgical and vision-care landscape at the time of the AMO deal was a tight oligopoly. In cataract surgery and IOLs, Alcon (Novartis subsidiary) was the global leader with the AcrySof IOL and Centurion phaco system; Bausch & Lomb (Valeant) and Carl Zeiss Meditec held meaningful share; AMO held the #2 IOL position with the TECNIS family and Catalys femtosecond laser. In laser refractive surgery, AMO's iLASIK / iDESIGN platform competed with Alcon (WaveLight) and Bausch & Lomb (Technolas). In consumer eye health, AMO's Blink and Complete contact-lens care products competed with Alcon's Opti-Free and Bausch & Lomb's renu/Biotrue. J&J's existing Vision Care business was contact lens-only (ACUVUE, the global #1). Acquiring AMO immediately created Johnson & Johnson Vision, vaulting J&J from a single-segment lens player into a full-stack ophthalmic franchise spanning surgery, refractive, and consumer eye health — the only credible scale challenger to Alcon. With ~3.8M annual U.S. cataract procedures and a global cataract device market of ~$7.1B in 2024 growing high single digits, the deal was framed as a long-duration demographic play on aging populations and elective surgery growth.
