Colorectal cancer is the third most commonly diagnosed cancer in the US, with ~95% of tumors classified as microsatellite stable (MSS) — the subset that does not respond to single-agent checkpoint inhibitors. Stage IV MSS CRC carries a poor prognosis and remains a major immuno-oncology unmet need because the tumors are 'cold,' lacking the T-cell infiltration needed for PD-1 blockade alone to work.
MSS metastatic CRC standard of care at deal time was chemotherapy backbones (FOLFOX/FOLFIRI plus bevacizumab or cetuximab/panitumumab depending on RAS status) and later regorafenib or trifluridine/tipiracil. Therapeutic cancer vaccines have a long failure history (Provenge being the partial exception) — most prominent attempts include Targovax's TG02, Gritstone bio's neoantigen vaccines, BioNTech/Genentech's autogene cevumeran, and Moderna/Merck's mRNA-4157 (now broader cancer indication). KISIMA's differentiator is a self-adjuvanted recombinant fusion protein that delivers multi-antigen cargo plus a TLR peptide agonist in one construct, combined with anti-PD-1 (Boehringer's ezabenlimab/BI 754091) to convert cold tumors hot. The €325M deal slots ATP128 into Boehringer's three-pillar oncology strategy alongside ViraTherapeutics oncolytic virus and OSE Immunotherapeutics SIRP-α antibody. Phase 1b KISIMA-01 read out tolerable with immunogenicity signals but no commercial momentum has followed.
