Disease Overview

Alzheimer's disease is a progressive neurodegenerative disorder characterized by amyloid-beta plaques, tau tangles, cholinergic deficit, and progressive cognitive decline. Despite affecting millions of Americans, the standard of care in 2016 was limited to symptomatic acetylcholinesterase inhibitors (donepezil, rivastigmine, galantamine) and memantine, all of which provided modest, transient cognitive benefit without disease modification.

Competitive Landscape

The Allergan-Heptares deal (April 2016) was struck one day after the Pfizer-Allergan $160B merger collapsed, and represented Allergan's pivot toward standalone CNS pipeline-building. The acquired portfolio—muscarinic M1, M4, and M1/M4 agonists including HTL9936 and HTL18318—addressed cognitive and behavioral symptoms via cholinergic stimulation, a mechanism class previously failed by Eli Lilly's xanomeline (which Karuna later resurrected as KarXT for schizophrenia). The 2016 competitive set was crowded but disappointing: Eli Lilly's solanezumab (anti-amyloid), Biogen/Eisai's aducanumab (later Aduhelm, controversially approved 2021 then withdrawn), Merck's verubecestat (BACE inhibitor, terminated 2017), and Roche's gantenerumab were the leading disease-modifying programs. Heptares' StaR-protein-derived selective M1/M4 agonists were positioned as cognitive-symptom enhancers without the M2/M3 cardiovascular and GI toxicities that derailed prior muscarinic programs. The lead asset HTL0018318 was suspended in September 2018 after toxicology findings (tumors at supratherapeutic doses) in nonhuman primates. AbbVie acquired Allergan in May 2020 and terminated the Heptares licensing agreement in January 2021, returning rights to Sosei Heptares.
