Disease Overview

Parkinson's disease (PD) is the second most common neurodegenerative disorder, affecting approximately 1 million Americans and nearly 10 million patients globally, with US incidence of approximately 90,000 new diagnoses annually (Parkinson's Foundation 2022 Prevalence Project, updated 2023). All currently approved therapies are symptomatic — predominantly dopamine replacement (levodopa / carbidopa combinations, dopamine agonists, MAO-B and COMT inhibitors) with adjunctive device-based advanced therapies (Duopa intestinal gel, deep brain stimulation, focused ultrasound, and newer subcutaneous apomorphine/Vyalev infusion). No approved drug demonstrably slows disease progression. The symptomatic PD market is estimated at $5.5B globally (2023 IQVIA). Genetic PD (LRRK2, GBA, PINK1, Parkin, alpha-synuclein) has become the organizing principle for disease-modification dealmaking, with multiple programs advancing on kinase modulation and protein aggregation. PINK1 and Parkin together constitute the mitophagy quality-control pathway — loss-of-function mutations cause autosomal recessive early-onset parkinsonism, and pharmacologic PINK1 activation is hypothesized to rescue defective mitophagy in sporadic PD. Mitokinin's lead program is a first-in-class oral PINK1 kinase activator targeting this mechanism.
