B cell-mediated autoimmune diseases are a heterogeneous group of chronic immune disorders in which pathogenic B cells drive autoantibody production, antigen presentation, and pro-inflammatory cytokine release. Flagship indications include systemic lupus erythematosus (SLE), idiopathic inflammatory myopathies (myositis), systemic sclerosis (scleroderma), and ANCA-associated vasculitis. Despite meaningful progress with biologics such as rituximab, belimumab, and anifrolumab, a large fraction of patients cycle through multiple immunosuppressants without achieving durable, drug-free remission. The therapeutic hypothesis advanced by ex vivo CD19 CAR-T programs in autoimmune disease (notably the Schett group data) is that deep B cell depletion resets the autoimmune repertoire and yields prolonged remission. The commercial problem is that ex vivo CAR-T requires apheresis, lymphodepletion, and inpatient monitoring, which makes it inaccessible for chronic, non-life-threatening autoimmune populations. In vivo CAR-T delivered via targeted lipid nanoparticles (tLNP) aims to collapse that access gap by reprogramming B cell-depleting T cells inside the patient as an off-the-shelf infusion.
The competitive MOA landscape in B cell-directed autoimmunity is crowded but fragmented by delivery modality. Approved anti-CD20 and BAFF-directed biologics (rituximab/Rituxan, ocrelizumab/Ocrevus in adjacent indications, belimumab/Benlysta, anifrolumab/Saphnelo targeting IFN-alpha) offer partial depletion but not durable reset. Ex vivo autologous CD19 CAR-T programs dominate the 'deep reset' narrative: Cabaletta Bio's rese-cel (CABA-201), Kyverna Therapeutics' KYV-101, Bristol Myers Squibb's Breyanzi (lisocabtagene maraleucel) in autoimmune exploration, and Novartis's rapcabtagene autoleucel (YTB323) are all in early-to-mid clinical studies for SLE, myositis, and scleroderma. T cell engagers are a second wave: Cullinan Therapeutics' CLN-978 (CD19xCD3 TCE), Genmab/AbbVie's epcoritamab (Epkinly) being explored in autoimmunity, and Regeneron's odronextamab. In vivo CAR-T is the newest competitive front where Capstan's CPTX2309 (tLNP-delivered anti-CD19 CAR mRNA) sits alongside Umoja Biopharma's VivoVec (lentiviral), Orbital Therapeutics' RNA-LNP, Kelonia Therapeutics' in vivo gene placement, and Moonlake Immunotherapeutics adjacent approaches. AbbVie's thesis is that tLNP manufacturability plus off-the-shelf administration will beat autologous CAR-T on access economics in non-oncology indications.
